Our Clinical Coaching Programme uses artificial intelligence (AI) to find patients who could benefit from extra support in managing their health
Our goal is to offer consistent and helpful support to improve you health.
If you are selected, here is what you can expect:
Initial Contact
HN will contact you to discuss the programme and see if you are interested in participating. You may receive a text message and/or a letter too.
Setting Up a Call
If you choose to take part, we will schedule a call with a Clinical Coach.
First Coaching Call
Your Clinical Coach, a qualified healthcare professional, will provide personalised guidance and work with you to create a health plan tailored to your needs.
Ongoing Support
You will receive regular check-ins and advice to help you stay on track, manage your health effectively, and reduce the need for emergency care.

How does it work?
Patient visits the Doctor
When you visit your GP, hospital, or ambulance service, routine health data is collected and combined.
Patient list
The NHS shares a list of patients' names and NHS identification (ID) based on the AI's recommendations.
Invitation call
You choose to take part or decline.
Data is sent to the AI
NHS removes all identifiable data before sharing to ensure your privacy.
Screening
HN's qualified clinical team review patient records to decide who might benefit from the service.
Coach assigned
If you join, you will be matched with a clinical coach and your first appointment will be booked.
AI runs on anonymous data
AI finds people who could find coaching useful.
Invitation message
You will receive more information about the programme.
Coaching starts
Your coaching sessions will start and can continue for up to six months.
